Studio Pulse Membership Agreement

This Membership Agreement ("Agreement") is entered into between Studio Pulse LLC, located at 600 N Shepherd Dr # 449, Houston, TX 77007, ("Studio Pulse") and the undersigned member, ______("Member").

Membership Details:

Membership Type: The Member agrees to purchase a membership at Studio Pulse.
Membership Benefits: This membership grants the Member access to ______ per month at Studio Pulse Austin.
Membership Fees: The recurring monthly fee for this membership is _____, which will be automatically charged to the Member’s credit card or other authorized payment method on a monthly basis.
Membership Start Date: The membership will start on the date of activation- ____.

Terms and Conditions:

Payment Authorization: By signing this Agreement, the Member authorizes Studio Pulse to charge the agreed-upon membership fee to the provided payment method each month, starting on ______.
Membership Duration: This Agreement is effective as of _____and shall continue on a month-to-month basis until terminated by either party in accordance with the cancellation policy outlined below.
No Show/Late Cancel Fee: The Member agrees to be automatically charged for any No Show or Late Cancel based on Studio Pulse’ cancellation policy. Cancellations made less than 8 hours before class start time will result in a fee of $10. Any no show reservations, including not checking in for your class at the start will result in a $15 no show fee.
Cancellation Policy: The Member may cancel their membership at any time through their online account. Cancellations will take effect at the end of the current billing cycle. No refunds or prorated refunds will be issued for the remainder of the month after a cancellation. To cancel a membership, the Member must submit the cancellation through their account via the Studio Pulse app or website. Cancellations will not be accepted via phone, email, or in person. It is the Member’s sole responsibility to initiate and confirm cancellation.
Use of Facilities: The Member agrees to adhere to all posted rules, guidelines, and safety protocols at Studio Pulse. Failure to comply may result in the suspension or termination of membership without refund.
Health and Safety Acknowledgment: The Member affirms that they are in good physical condition and know of no medical reason why they should not participate in Pilates or other exercise activities. Participation in all classes is at the Member's own risk.
Modification of Terms: Studio Pulse reserves the right to modify the terms of this Agreement, including membership fees and benefits, with 30 days' notice to the Member. Notices will be sent via email to the address provided by the Member.

Miscellaneous:

Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the State of Texas.
Entire Agreement: This Agreement represents the entire understanding between the parties and supersedes any prior agreements or understandings related to the subject matter hereof.
